Health insurance which is compulsory to get the residence permit,

ID card information form,

Transcript,

1 jpeg photo fort he student ID card,

10 passport sizes photos,

Passport copy.

Language level certificate (TOEFL,ELTS etc. або інший сертифікат)

 

The documents are required signed and stamped should be sent by e-mail first then the originals by post,
